Registration settings not always work

Hi all,

I'm experiencing something weird: although I set the registration to the forum to be approved by the admin, sometimes someone gets a member status directly - the admin not even being notified by this.
The setting for registration are not changed, yet the process is skipped.
The most annoying is that the given email address seems a whatever, so all looks as a spam...

Any advice on this?
